Fever of Unclear Origin and Cytopenia Because of Acute Splenic Sequestration in a Young Immunocompetent Carrier of Beta-Globin Mutation for Hb Vallet…

2008

Fever of unclear origin is a clinical challenge in medical practice. Infectious diseases, neoplasms, and collagen vascular illnesses are its main causes in adults and children. Acute splenic sequestration crises, a known potentially fatal complication of sickle cell disease and sickle beta-thalassemia, are uncommon in beta-heterozygosis. We describe a case of prolonged recurrent episodes of fever with spontaneous resolution, commencing at age 10 in a 15-year-old boy with a history of hypochromic microcytic anemia attributed to a thalassemic trait. He was admitted twice to our university hospital for continuous-remittent fever with a pruritic, macular evanescent Still's skin rash, severe spl…

Anemia of Chronic Disease: Pathophysiology and Laboratory Diagnosis

2005

Classic iron deficiency (ID) does not represent a challenge for the laboratory and physicians. The anemia that accompanies infection, inflammation, and cancer, commonly termed anemia of chronic disease (ACD), features apparently normal or increased iron stores. However, 20% of these patients have iron-restricted erythropoiesis (functional ID), an imbalance between the iron requirements of the erythroid marrow and the actual iron supply. Functional ID leads to a reduction in red cell hemoglobiniza-tion, causing hypochromic microcytic anemia. The diagnosis of functional ID in real time is based on measuring the hemoglobin content of reticulocytes. An examination of the biochemical markers of …

756 The Effectiveness of RDW-CV in Differentiating Microcytic Anemia

2012

Backround: Iron deficiency anemia (IDA) and thalassemia are the most common microcytic anemia in children. Sometimes, expensive or invasive investigations are necessary for their distinction. Red cell distribution - coefficient variation (RDW-CV) is a new generation marker more often used in differentiating IDA from thalassemia. A number of hematological indices are also useful (Mentzer, Shine&Lal, Green&King, Ricerca). Aim To demonstrate the effectiveness RDW - CV in differentiating microcytic anemia. Method Retrospective study conducted on a total of 215 patients with microcytic anemia. Criteria for study group selection: MCV and Hb below the minimum age reference (mild anemia, Hb between…
